SUPPORT WE PROVIDE
Support Connection
Level 1 Support: We will assist to build your ability to connect with informal, community and funded supports.
Support Coordination
Level 2 Support: We will work with you to ensure appropriate supports are in place to increase your capacity to maintain relationships, manage daily tasks, access your community and live more independently.
Specialised Support Coordination
Level 3 Support: Higher Level support provided by qualified specialists to assist you in managing challenges in your support environment, while ensuring consistent delivery of services.
